A fair physiotherapy commission scheme pays for two things separately: sales commission on the money the centre actually collects for a session package, calculated in tiers, and hands-on commission each time a therapist completes a session. Therapists are then rewarded for the treatment they deliver, the centre never pays for sessions that have not happened, and nobody is pushed to sell more sessions than a patient needs.
Why commission is tricky in a physiotherapy centre
Physiotherapy centres sell care in packages, and that creates commission questions a simple per-bill scheme cannot answer:
- Packages are paid before the work is done. A patient may pay for a package of sessions after the first assessment, then attend over several weeks.
- The person who recommends a package is not always the one who treats. A senior therapist may plan the treatment while another therapist carries out some of the sessions.
- Patients pay in parts. Some patients pay for a package in instalments, so the money arrives over time.
- Patients stop coming. A package can be left half-used when a patient feels better or gets busy, and those sessions are still owed.
- Trust matters more than sales. If commission rewards selling bigger packages, patients may feel pressured, and the centre’s reputation suffers.
Physiotherapy runs on patients’ trust. If commission only rewards selling bigger packages, the centre’s reputation is what suffers in the end. Reward therapists for each session they complete, and everyone’s focus returns to the patient finishing the treatment plan.

Two tracks: one for the package, one for each session

1. Sales commission: on money actually collected, in tiers
Pay sales commission on total collection, not on the package value written on the bill. If a patient pays in parts, the person who sold the package earns as each payment comes in. Tiers reward the staff who bring in more collections, and the centre never pays commission on money it has not received.
2. Hands-on commission: every time a session is completed
Pay hands-on commission each time a therapist completes a session or a session is redeemed from a package. If a package is shared between two therapists, each one earns for the sessions they actually treated, and sessions that are never used do not cost the centre commission.
3. Put the treatment plan first
Commission should follow the work, not the size of the package. Rewarding completed sessions keeps the focus on patients finishing their plan, rather than on selling more sessions than they need.
Rules to write down before the next pay day
- Which items earn sales commission. Session packages, single sessions, products, and who counts as the seller when a package is recommended by one therapist and paid at the front desk.
- Which sessions earn hands-on commission. List each treatment type, so every therapist reads the same rule.
- What happens when a patient changes therapist. Hands-on commission goes to whoever completes each session; sales commission stays with the seller.
- How part-payments are treated. Sales commission follows each payment received.
- Where therapists can check. Show each therapist the sessions they completed, so pay day is not a debate.
To keep each patient’s remaining sessions, balance and follow-ups under control, read physiotherapy package management. For commission structures that apply to every kind of salon, see our salon staff commission guide.
Common mistakes
- Paying all commission on the day the package is sold. The centre pays for sessions that may never take place.
- Paying on the bill amount when the patient has not paid in full. Commission runs ahead of the money.
- One flat rate for everything. Selling and treating are different jobs and deserve different rules.
- Working it out in a spreadsheet at month end. Errors and arguments arrive together.
How MyBMS Pro calculates therapist commission
MyBMS Pro is an AI-driven, next-generation SaaS management hub specifically designed for multi-branch beauty salons. Its two-track tiered commission engine fits the way physiotherapy packages are sold and used:

- Sales commission in tiers on total collection. Commission is calculated on the money actually collected, not on the bill value.
- Hands-on commission when a session is redeemed. Commission is triggered when a package session is redeemed or a service is completed, for the therapist who performed it.

FAQ
How should a physiotherapy centre pay therapist commission?
Separate selling from treating. Pay sales commission on the money actually collected for packages, in tiers, and pay hands-on commission each time a therapist completes a session.
Do physiotherapists get commission when a package is sold or when each session is done?
Both, for different jobs. Sales commission follows the payments received for the package. Hands-on commission is paid session by session, when each session is completed or redeemed.
If two therapists treat the same patient, who gets the commission?
Each therapist earns hands-on commission for the sessions they actually completed. The person who sold the package keeps the sales commission.
What if a patient pays for the package in parts?
Pay sales commission as each payment is received, so commission never runs ahead of the money the centre has collected.
Is there a MyBMS Pro mobile app?
No. MyBMS Pro is a web-based system used in the browser.
